Job summary

Cadence Health seeks a bilingual Spanish-speaking Clinical Navigator to deliver virtual chronic care support for Medicare patients enrolled in Cadence programs. The role involves coaching, care coordination, and documentation within the Cadence platform, with a focus on education, goal setting, and addressing barriers to care.

You will work remotely from the United States, with standard hours Mon-Fri and time-zone options across Mountain, Pacific, Central regions.

Qualifications

• NBHWC health coaching certification is required.
• At least 1 year of experience as a health coach is required.
• Experience providing patient education and support to individuals with chronic conditions is required.
• Remote work readiness (stable high-speed WiFi) is required.
• Fluency in Spanish is required.
• Fluency with AI-assisted clinical tools and ability to discuss AI support vs. human judgment.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching to patients with chronic conditions.
• Coach patients on behavior modification, nutrition, physical activity, and self-management using evidence-based techniques.
• Perform comprehensive assessments of health status and readiness to change.
• Educate patients on disease management and medication adherence, following Cadence policies.
• Monitor progress and health outcomes via remote check-ins and monitoring tools.
• Document interactions and coordinate care with clinical teams in Cadence platform and other tools.
