Job summary

Cadence is seeking a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. You will conduct outbound outreach, coaching, and care coordination to close gaps in care, documenting interactions in the Cadence platform while collaborating with clinicians.

Ideal candidates have an active LPN/LVN license, 5+ years of clinical nursing, strong remote-work readiness, and Spanish fluency.

Qualifications

• Active LPN/LVN license in the state of residence.
• Minimum 5 years of clinical nursing experience.
• Remote work readiness with stable high‑speed internet.
• Spanish fluency preferred.
• Experience educating and supporting patients with chronic conditions.

Responsibilities

• Outbound patient outreach and one‑on‑one coaching for chronic conditions.
• Educate and coach on behavior change, nutrition, activity and self‑management.
• Conduct health status assessments and readiness to change.
• Educate on disease management, adherence, and prevention strategies.
• Monitor progress and document outcomes in the Cadence platform.
• Coordinate with clinicians and support teams in real time.
• Identify barriers to care and connect patients with resources.
