Job summary

Cadence is seeking a Bilingual/Spanish Speaking Licensed Practical Nurse to deliver virtual chronic care support for Medicare patients in Cadence programs. This full-time, remote position requires an LPN with Spanish fluency to conduct patient outreach, coaching, and care coordination, documenting interactions in the Cadence platform.

Qualifications

• Active multi-state LPN/LVN license required.
• Minimum 5 years of clinical nursing experience required.
• Remote work readiness with reliable high-speed internet.
• Active CA LVN license is highly preferred.
• Spanish fluency required.

Responsibilities

• Conduct outbound telephonic patient outreach and provide coaching to patients managing chronic conditions.
• Educate and coach patients on personalized care plans, nutrition, activity, and self-management strategies.
• Perform comprehensive health and lifestyle assessments and document interactions in the Cadence platform.
• Coordinate care with clinicians and support teams to address barriers to care.
• Monitor progress and outcomes through remote check-ins and Cadence tools.
