Job summary

Cadence is seeking a bilingual Spanish-speaking Clinical Navigator to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. This full-time, remote role is based in the United States with standard hours Monday–Friday.

You will conduct outbound outreach, coach patients on behavior change, manage care plans, coordinate with clinicians, document interactions, and use Cadence Platform, Google Workspace, and Slack.

Qualifications

• NBHWC certification is required.
• At least 1 year of health coaching experience.
• Experience educating and supporting patients with chronic conditions.
• Remote work readiness with stable high-speed WiFi.
• Spanish fluency preferred.
• Familiarity with AI-assisted clinical tools preferred.

Responsibilities

• Outbound patient outreach and one-on-one coaching for chronic conditions.
• Educate and coach on behavior modification, nutrition, activity, and self-management.
• Assess health status, readiness to change, and lifestyle.
• Educate on disease management and medication adherence; follow Cadence protocols.
• Monitor progress and document interactions in Cadence platform.
• Collaborate in real time with clinicians using Cadence Platform, Google Workspace and Slack.
