Job summary

Cadence is hiring a bilingual Clinical Navigator (LPN) to provide virtual chronic care support for Medicare patients in Cadence care programs. You’ll conduct proactive outreach, coaching, and care coordination to close gaps in care, addressing barriers such as resources, transportation, or refills.

You will document patient interactions, collaborate with clinicians, and use Cadence Platform, Google Workspace, and Slack to elevate concerns and coordinate next steps while ensuring patients feel

Qualifications

• Active compact multi-state LPN/LVN license in the state of residence.
• Minimum 5 years of clinical experience as an LPN/LVN required.
• Remote work readiness with reliable high-speed WiFi.
• Active CA LVN license highly preferred.
• Fluency in Spanish required; AI-tool familiarity a plus.

Responsibilities

• Conduct outbound patient outreach and coaching for chronic conditions.
• Educate and support patients to follow personalized care plans.
• Assess health status, readiness to change, and adherence.
• Document interactions and outcomes in Cadence platform with supervision.
• Collaborate in real time with clinicians using Cadence, Google Workspace and Slack.
