Job summary

Cadence is seeking a bilingual Spanish-speaking Clinical Navigator (Health Coach) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. This full-time remote role is based in the United States with standard hours Monday–Friday across Pacific and Mountain time zones.

You will coach patients with chronic conditions, educate on self-management, and collaborate with clinicians using Cadence Platform, AI tools, and other apps.

Qualifications

• Active NBHWC health coaching certification is required.
• At least 1 year of experience as a health coach is required.
• Experience providing patient education and support to individuals managing chronic conditions is required.
• Remote work readiness (including stable high-speed WiFi) is required.
• Fluency in Spanish is strongly preferred.
• Fluency with AI-assisted clinical tools (e.g., Gemini, Open Evidence) and ability to discuss AI support vs human judgment.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions.
• Help patients execute personalized care plans and achieve goals using education and coaching on behavior modification, nutrition, physical activity, and self-management with motivational interviewing and SMART goals.
• Conduct comprehensive assessments of health status, lifestyle behaviors, and readiness to change.
• Educate patients on disease management, medication adherence, symptoms, and prevention per Cadence policies.
• Monitor progress and health outcomes through regular check-ins and remote monitoring tools.
• Document patient interactions and outcomes in the Cadence platform and connect patients with resources as needed.
• Collaborate in real time with clinicians and patient support teams using Cadence Platform, Google Workspace, and Slack.
