Cadence is seeking a bilingual Spanish-speaking Clinical Navigator (Health Coach) to deliver virtual chronic care support for Medicare patients in Cadence programs. This full-time remote role is based in the United States, with standard hours across multiple time zones.

You will conduct outreach, coach patients on behavior change, educate on disease management, monitor progress, and document interactions while collaborating with clinicians using Cadence tools and platforms.

Competenze

• NBHWC health coaching certification is required.
• At least 1 year of health coaching experience.
• Experience educating and supporting patients with chronic conditions.
• Remote work readiness with stable high-speed internet.
• Spanish fluency is required.
• Familiarity with AI-assisted clinical tools is a plus.

Mansioni

• Conduct outbound telephonic patient outreach and coaching.
• Assist patients with personalized care plans and behavior change goals.
• Assess health status, lifestyle, nutrition, readiness to change.
