Job summary

Cadence, a clinical AI company, is seeking a bilingual Medical Assistant to deliver remote chronic care support for Medicare patients. You will conduct outreach, coach patients, and coordinate care, documenting interactions in the Cadence platform while collaborating with clinicians.

The role requires MA certification, 5+ years experience, Spanish fluency, and readiness for remote work. The position is fully remote in the United States with standard M-F hours.

Qualifications

• Active Medical Assistant certification from an accredited association is required.
• At least 5 years of experience as a Medical Assistant is required.
• Remote work readiness (stable high-speed WiFi) is required.
• Fluency in Spanish is required.
• Experience with AI-assisted clinical tools and discussing AI supports with providers/patients is preferred.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions.
• Educate and coach patients on behavior modification, nutrition, activity, and self-management strategies.
• Conduct comprehensive health status assessments and readiness to change.
• Educate on disease management, medication adherence, symptoms, and prevention; follow Cadence policies.
• Monitor progress and health outcomes via check-ins and remote tools.
• Document interactions and outcomes in Cadence platform; identify barriers to care and connect to resources.
• Collaborate in real time with clinicians and patient support teams; provide feedback to improve workflows.
