Job summary

Cadence is a clinical AI company delivering virtual chronic care support for Medicare patients enrolled in Cadence care programs. We are seeking a Bilingual/Spanish Speaking Clinical Navigator (Medical Assistant) to conduct outreach, coaching, and care coordination to close gaps in care.

The role involves documenting interactions in the Cadence platform, collaborating with clinicians, and guiding patients through personalized care plans with evidence-based coaching.

Qualifications

• Active Medical Assistant certification from an accredited association.
• At least 5 years of experience as a Medical Assistant.
• Remote work readiness with stable high-speed WiFi.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions.
• Help patients execute personalized care plans and achieve goals through education, coaching, and behavior change strategies.
• Perform comprehensive assessments of health status, lifestyle, and readiness to change.
• Educate patients on disease management, medication adherence, symptoms, and prevention per Cadence policies.
• Monitor patient progress and outcomes via check-ins and remote monitoring tools.
• Document patient interactions, outcomes, and follow-up plans in the Cadence platform; identify barriers and connect patients with resources.
• Collaborate in real time with clinicians and support teams using Cadence Platform, Google Workspace, and Slack.
