Job summary

Cadence is hiring a bilingual Spanish-speaking Clinical Navigator to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. You will conduct outbound outreach, coaching, and care coordination to close gaps in care.

This full-time remote role requires NBHWC certification, 1+ year health coaching experience, Spanish fluency, and comfort using AI-assisted clinical tools. The position offers competitive pay, full benefits, and a home office stipend.

Qualifications

• NBHWC certification is required.
• At least 1 year of health coaching experience is required.
• Remote work readiness (stable, high-speed WiFi) required.
• Spanish fluency required.
• Fluency with AI-assisted clinical tools and ability to discuss AI support vs. human judgment with providers/patients.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one‑on‑one coaching to patients with chronic conditions.
• Provide education and coaching on behavior modification, nutrition, physical activity, and self-management strategies.
• Assess patients' health status, lifestyle, and readiness to change.
• Educate patients on disease management, medication adherence, and prevention strategies; follow Cadence policies.
• Monitor progress and outcomes; document interactions and follow-up plans.
