Job summary

Cadence is seeking a bilingual Spanish-speaking Clinical Navigator (Health Coach) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. This full-time remote role is based in the United States, with standard hours Mon-Fri 8-5 PT or 9-6 MT.

You will conduct outbound outreach, coach on behavior change, and support disease management while documenting care plans in the Cadence platform.

Qualifications

• NBHWC Health Coaching Certification required.
• At least 1 year of health coaching experience.
• Experience educating patients with chronic conditions.
• Remote work readiness (stable high-speed WiFi) required.
• Spanish fluency strongly preferred.
• Familiarity with AI-assisted clinical tools and platforms.

Responsibilities

• Outreach via phone; provide one-on-one coaching for chronic conditions.
• Educate on behavior change, nutrition, activity, self-management.
• Assess health status and readiness to change.
• Educate on disease management, adherence, prevention; follow Cadence policies.
• Monitor progress and outcomes; document interactions.
• Collaborate with clinicians and support teams in real time.
