Responsibilities

• Patient Care - Age Specific and Culturally Inclusive: Integrate age-specific and culturally inclusive concepts into patient care.
• Interdisciplinary Collaboration: Promote patient/family well-being and coordinate with the health care team and outside agencies.
• Psychosocial Assessment: Conduct comprehensive psychosocial assessments to identify needs and safety concerns.
• Psychosocial Interventions: Provide psychosocial services and counseling to improve coping and adherence to treatment plans.
• Resource Development and Utilization: Develop networks and collaborate with community resources to meet patient needs.
• Leadership: Serve as a clinical expert, provide team leadership and participate in staff development and evaluation.
• Compliance: Ensure practice conforms to regulations and maintain professional competency.

Education

Master's degree in Social Work or Counseling

5+ years of work experience in a related job discipline

Current independent Social Work or LPCC license to practice in the State of Ohio
