Qualifications

• Bachelor’s degree and 2+ years’ experience or Master’s degree with 1 year experience in one or more of the following areas: Family systems; Community systems and resources; Case management; Child and Family counseling or therapy; Child protection; Child development.

Responsibilities

• Care Delivery: Develop and implement an individual crisis and safety plan with youth and family; facilitate referrals and linkages; monitor plan implementation; manage 1915(c) Waiver funds with prior authorization. Ensure culturally competent services and family-friendly supports. Conduct outreach, assessments, and meetings in person or virtually.
• Plan of care: Implement and document responses to individualized care plans; engage families; develop CFCCP with the team; lead CFT meetings monthly; complete CANS assessments and trauma-informed care approaches.
• Therapeutic Relationships/Communication: coach staff; facilitate CFT and CFCCP development; coordinate with providers and school personnel; share progress and secure approvals; explore community resources to meet youth and family needs.
