Qualifications

• Master's degree in Social Work or Counseling.
• Current independent Social Work license to practice in Ohio (LPCC).
• 5+ years of work experience in a related field.

Responsibilities

• Patient Care - age-specific and culturally inclusive practices integrated into patient care.
• Interdisciplinary collaboration to promote patient/family well-being and coordinate care.
• Psychosocial assessment to identify needs and plan interventions.
• Provide psychosocial interventions and self-management support for high-risk families.
• Develop resource networks and advocate for patient populations at the system level.
• Leadership and compliance, including staff coordination and adherence to standards.
