Qualifications

• Active license (LSW/LISW or RN) or a Bachelor's in Nursing or Social Services with 2 years of direct service experience.
• Ability to work independently within guidelines and maintain documentation.
• Knowledge of Medicaid/Medicare, waivers, and community resources.

Responsibilities

• Guide and supervise Care Coordinators, focusing on clinical issues, staff development, and caseload management.
• Maintain knowledge of Medicaid, behavioral health systems, and community resources.
• Develop and monitor person-centered care plans aligned with member needs and preferences.
• Connect individuals to services, including medical, social, and educational supports.
• Document care coordination activities accurately and timely per program guidelines.
