Job summary

GlobalHealth, Inc. is seeking a Care Manager to perform telephonic case management under the direction of the Supervisor, Care Management. You will advocate for members, coordinate care, and collaborate with providers to drive optimal outcomes.

Responsibilities include SNP MOC activities, post-discharge follow-up, patient education, and ensuring access to follow-up care. Remote work with HIPAA compliance is required.

Qualifications

• Active RN license in Oklahoma required.
• Active multistate RN license preferred.
• Experience in managed care/utilization management preferred.
• Knowledge of nursing processes, techniques, and standards.
• Clear oral and written communication skills.

Responsibilities

• Conduct telephonic case management for complex, high-risk members, identifying needs and coordinating care.
• Coordinate with providers to ensure holistic, beneficial outcomes.
• Communicate with diverse member populations respecting culture, language, and beliefs.
• Advocate for patients, provide crisis intervention and ensure quality care.
• Coordinate services with community resources and care team members.
