This role bridges community members with healthcare services, helping individuals understand their needs and connect with providers, social services, and wellness programs.

Qualifications

• Experience in community health, social work, or a related field.
• Strong interpersonal, communication, and motivational interviewing skills.
• Knowledge of local health resources and the healthcare system.

Responsibilities

• Help community members identify health needs and access services.
• Provide information on preventative care and chronic disease management.
• Assist with insurance navigation, appointment scheduling, and transportation to facilities.
• Connect individuals with social services, mental health resources, and wellness programs.
• Conduct health education outreach to promote healthy lifestyles.
