You will assess needs, develop plans, and connect clients with community resources while maintaining secure records and delivering compassionate, high-quality service.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Minimum of 3 years of experience in social work, counseling, or related support role.
• Experience conducting needs assessments and providing virtual support interventions.
• Excellent virtual communication, active listening, and empathy skills.
• Proficiency with teleconferencing software, CRM systems, and secure digital records.

Responsibilities

• Conduct remote assessments to identify clients' social, emotional, and practical support needs.
• Provide empathetic counseling and support via phone, video calls, and secure messaging platforms.
• Develop personalized support plans tailored to individual client circumstances and goals.
• Research and connect clients with relevant community resources, such as healthcare providers, educational programs, and financial aid services.
