Job summary

Placements24 is hiring a Remote Community Care Specialist to provide client-centered support across South Africa from a home office. You will guide clients in accessing social and health services, using digital tools, and maintaining case records with confidentiality.

The role requires at least 4 years in community support or related fields, excellent communication, and strong digital literacy. Work is fully remote with opportunities to engage communities nationwide.

Qualifications

• Bachelor-level or higher relevant qualification in_social work, community development, psychology, or related field.
• Minimum 4 years of experience in community support, social services, or case management, with remote interaction.

Responsibilities

• Provide remote, person-centred support via phone, video, and email.
• Conduct virtual needs assessments to identify client challenges and requirements.
• Assist clients in navigating social, health, and community services nationally.
• Develop personalised action plans and provide follow-up support.
• Maintain accurate, confidential client records in the digital case management system.
• Stay updated on resources and programs across SA regions.
• Liaise between clients and service providers for referrals.
• Develop and deliver online workshops on relevant topics.
• Collaborate with internal teams and external partners online.
• Troubleshoot client issues with empathetic, practical solutions.
