Job summary

Centene Corporation is seeking an experienced care management professional to oversee behavioral health and social determinant focused care plans for its members. The role emphasizes complex case review, cross-functional collaboration, and high-quality, cost-effective care delivery.

You will work remotely with North Carolina residency and occasional travel to Raleigh or Charlotte, following a standard M-F 8:00 AM to 5:00 PM schedule.

Qualifications

• Requires Master’s degree in Behavioral Health or Social Work or Nursing degree and 5–7 years of related experience.
• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN per state contract requirements with BH experience required.
• For Illinois Youth Care plan only: IL licensure requirements apply.

Responsibilities

• Oversees and reviews care plans including behavioral health and social determinants.
• Performs telephonic, digital, home, or site visits to assess member needs.
• Guides complex care plans and triages to appropriate behavioral health providers.
• Ensures compliance with state, federal, and payer regulations.
• Monitors outcomes and revises care plans based on member needs.
