Job summary

Centene is seeking a Senior Care Manager to develop, assess, plan, implement, and coordinate complex care management activities focused on behavioral health needs. This fully remote role combines clinical judgment with high-acuity care delivery.

You will manage care plans via phone and in-home visits, coordinate with providers, monitor outcomes, and document all activities. Requires a Master's in Behavioral Health or Social Work, 4–6 years of behavioral health experience, and an appropriate

Qualifications

• Master's degree in Behavioral Health or Social Work or nursing degree required.
• 4–6 years of related experience in behavioral health.
• Licensure: LCSW, LMSW, LMFT, LMHC, LPC or RN as per state contract.

Responsibilities

• Develop and continuously assess care plans for high-acuity members by phone or in-home visits.
• Coordinate care between members, families, and the care provider team.
• Monitor outcomes and identify barriers to care coordination.
• Review referrals and intake assessments to update plans.
• Collaborate with providers and partners to ensure smooth care delivery.
