Job summary

Centene Corporation is seeking a care management professional to support primarily behavioral health needs for members, including education for families and personalized care planning. The role involves up to 75% local travel, remote work from home for the remainder, and a Monday–Friday schedule.

The position requires a Master’s degree in Behavioral Health or Social Work, or a Nursing degree, plus 2–4 years of related experience.

Qualifications

• Requires a Master’s degree in Behavioral Health or Social Work or a Nursing degree from an accredited school.
• 2–4 years of related experience.
• Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN as per state contract requirements.

Responsibilities

• Evaluates member needs by phone or in-home visits to determine resources and creates care/service plans.
• Performs telephonic, digital, home, and other site visits to assess needs and coordinate resources.
• Develops ongoing care plans for high-acuity members and identifies providers and community resources.
• Coordinates between members/families, resources, and care teams to ensure services are accessible.
• Monitors and adjusts care plans based on treatment side effects, symptoms, and outcomes.
• Facilitates care coordination to ensure timely access to care or services.
• Documents and maintains member information to comply with state, federal, and payer regulations.
• Educates members and families on procedures, treatments, and benefits; supports behavioral health needs.
• Provides feedback to leadership to improve care delivery cost-effectively.
• Performs other duties and complies with policies.
