Lehigh Valley Health Network is seeking an experienced RN to provide triage and healthcare navigation via remote telephone support in Pennsylvania. You will assess patient needs, guide care decisions, and coordinate across labs, referrals, and care teams.

Required is an Associate’s Degree in Nursing with PA RN licensure and 5+ years of acute care/telephone triage experience. Strong computer skills, empathy, and confidentiality are essential for success in this remote role.

Qualifications

• Associate’s Degree in Nursing.
• 5+ years of nursing experience in acute care, ED, medical office, or telephone triage.
• 2+ years of clinical acute care hospital experience/urgent care.
• Colleagues advancing from LPN to RN within the department are eligible.
• Excellent computer skills and empathetic patient support.
• Knowledge of home health information for patients awaiting physician visit.
• Knowledge of telephone-based clinical assessment techniques and triage protocols.
• Ability to exercise judgement with confidential information.
• Skill in triage decision making and calm emergency response.
• RN licensure PA in Pennsylvania.

Responsibilities

• Provide triage telephone service to identify high-risk conditions and determine prompt care.
• Perform telephonic triage to guide callers to appropriate level of care.
• Coordinate care across settings including labs, studies, and referrals with documented communication.
• Direct callers to the correct level of care and offer healthcare education.
• Maintain and file patient records and reports in designated locations.
• Refill prescriptions and place orders for testing and labs as needed.
