• Deliver telephonic triage following approved guidelines and protocols.
• Coordinate care across settings, including labs, referrals, and follow-ups.
• Maintain and file patient charts and reports in designated electronic systems.
• Act as liaison among patients, families, clinicians, and hospital staff.
