Zusammenfassung

Call 4 Health is seeking an experienced Registered Nurse (RN) for remote triage across multiple states. You will provide timely clinical assessments by phone and portal, document in EHRs, and support providers with referrals, RPM programs, and care coordination from a home-based setting.

Ideal candidates have 2–4 years of RN experience (ER/urgent care or ICU preferred), an active compact license, and a willingness to obtain additional state licenses as needed.

Qualifikationen

• Associate’s or Bachelor’s Degree in Nursing required.
• 2–4 years of full-time RN clinical experience (ER/Urgent Care/Adult/Pediatric/ICU).
• Telephone triage experience with electronic triage systems preferred.
• Experience with two or more EHR platforms.
• Active, unencumbered ENCL / Enhanced Nurse Licensure Compact license.
• Willingness to obtain additional state licenses as requested.

Aufgaben

• Respond to inbound and outbound calls for multi-state nurse triage service.
• Use Schmitt-Thompson protocols to assess and triage patients.
• Document patient interactions in client EHRs and/or Call 4 Health platform.
• Provide clinical assessments via phone or patient portal per protocols.
• Assist with referrals, prior authorizations, and clinical input.
• Collaborate with hospitals, nursing homes, and agencies as needed.
• Participate in Remote Patient Monitoring (RPM) programs.
