Call 4 Health seeks an experienced Registered Nurse (RN) for remote triage across multiple states. You will perform telephone-based triage, document interactions in EHRs, and support clinicians with clinical assessments and referrals from a home office.

The role emphasizes patient education, adherence to triage protocols, and collaboration with hospitals and providers to coordinate care in a fast-paced, 24/7 environment.

Qualifications

• RN with Associate's or Bachelor's in Nursing.
• Active, unencumbered Compact license (e-NCL) preferred.

Responsabilités

• Respond to inbound/outbound calls for a multi-state nurse triage service.
• Utilize established triage protocols to assess and triage patients.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal per protocols.
• Support providers with administrative and clinical tasks (e.g., refills, critical results).
• Coordinate post-discharge and post-operative follow-ups; assist with referrals and prior authorizations.
• Collaborate with hospitals, nursing facilities, and other agencies as needed.
• Participate in remote patient monitoring, including vital signs and disease education.
