Job summary

Call 4 Health is seeking an experienced Registered Nurse (RN) for remote triage across multiple states. You’ll perform telephone triage, document in EHR systems, and support providers with clinical assessments from home.

The role requires an Associate’s or Bachelor’s in Nursing, 2–4 years of full-time RN experience (ER/urgent care or ICU preferred), and an active compact license with willingness to obtain additional state licenses. Work from home in a fast-paced national center.

Qualifications

• RN degree (Associate’s or BSN) is required.
• 2–4 years of full-time RN clinical experience, preferably in ER/Urgent Care, Adult, Pediatric, Ambulatory Care, or ICU.
• Prior telephone triage experience using electronic triage systems.
• Experience working with two or more EHR platforms.
• Active, unencumbered license (Compact license).
• Willingness to obtain additional state licenses as requested.

Responsibilities

• Respond to inbound and outbound calls for a multi-state nurse triage service.
• Utilize established triage protocols to assess and triage patients.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal.
• Support providers with administrative and clinical tasks, including medication refills and notification of critical results.
• Facilitate referrals and assist with prior authorization requests.
• Collaborate with hospitals, nursing homes, and other agencies as needed.
• Participate in Remote Patient Monitoring programs (vital signs, chronic disease education).
• Screen and qualify patients for clinical trials and provide education.
• Take on additional responsibilities to support team success and patient outcomes.
