Job summary

Call 4 Health is seeking experienced Registered Nurses to join our 24-hour remote triage call center. As a Remote Triage RN, you will assess patients by phone using established protocols, document interactions in EHRs, and support providers nationwide.

The role offers flexibility and meaningful patient impact from home, with shifts in ET and a requirement for an RN license under the Nurse Licensure Compact. You will collaborate with hospitals, clinics, and RPM programs to deliver timely care.

Qualifications

• Associate’s or Bachelor’s Degree in Nursing.
• 2–4 years of full-time RN clinical experience (ER/Urgent Care, Adult, Pediatric, Ambulatory Care, or ICU).
• Prior telephone triage experience using electronic triage systems.
• Experience working with two or more EHR platforms.
• Active, unencumbered e-NCL license (Nurse Licensure Compact).
• Willingness to obtain additional state licenses as requested.

Responsibilities

• Respond to inbound and outbound calls for a multi-state nurse triage service.
• Utilize triage protocols to assess and triage patients.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal.
• Support providers with administrative and clinical tasks, including medication refills and notification of critical results.
• Coordinate post-discharge and post-operative follow-ups.
• Facilitate referrals and assist with prior authorizations.
• Collaborate with hospitals, nursing homes, and other agencies as needed.
• Participate in Remote Patient Monitoring (RPM) programs and educate patients.
• Screen and qualify patients for clinical trials and provide education.
• Take on additional responsibilities to support team success.
