Job summary

Call 4 Health is seeking an experienced Registered Nurse (RN) to join our 24-hour remote triage call center. The RN will perform telephone triage, EHR management, and clinical support across multiple states from a home-based setting.

This role emphasizes clinical decision-making, timely patient assessments, and collaborative work with providers to support patient care while offering flexibility and meaningful impact.

Qualifications

• Nursing degree (Associate’s or Bachelor’s) required.
• 2–4 years of full-time RN clinical experience.
• Prior telephone triage experience preferred.
• Experience with two or more EHR platforms.
• Active, unencumbered e-NCL/Enhanced Nurse Licensure Compact license; willingness to obtain additional state licenses.

Responsibilities

• Respond to inbound and outbound calls for a multi-state nurse triage service.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal based on established protocols.
• Support providers with administrative and clinical tasks, including medication refills and notifying critical results.
• Coordinate post-discharge and post-operative follow-up; facilitate referrals and prior authorization requests with clinical input.
• Collaborate with hospitals, nursing homes, and government agencies as needed.
• Participate in Remote Patient Monitoring programs, including vital signs and disease management education.
• Screen and qualify patients for clinical trials and provide patient education.
