Call 4 Health is seeking an experienced Registered Nurse (RN) to join our 24-hour medical call center as a Remote Triage Nurse. You will assess patients by phone, perform triage using established protocols, document interactions in client EHR systems, and support providers across multiple states.

This role suits nurses who excel in fast-paced environments, appreciate clinical decision-making, and want flexibility without compromising patient care.

Qualificações

• Associate’s or Bachelor’s Degree in Nursing.
• 2–4 years of full-time RN clinical experience.
• Compact license (eNLC) required; willingness to obtain additional state licenses.

Responsabilidades

• Respond to inbound and outbound calls for a multi-state nurse triage service.
• Utilize triage protocols to assess and triage patients.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal per protocols.
• Support providers with administrative and clinical tasks, including medication refills and post-discharge follow-up.
• Facilitate referrals and assist with prior authorization requests.
• Collaborate with hospitals, nursing homes, and government agencies as needed.
• Participate in Remote Patient Monitoring and educate patients on chronic disease management.
