Job summary

Call 4 Health is seeking experienced Registered Nurses for remote triage roles. As a Remote Triage Nurse, you will handle calls from patients nationwide, perform clinical assessments, and document interactions in EHR systems.

You will collaborate with providers and support RPM programs from a home-based setup. The role emphasizes fast-paced, flexible scheduling with shifts in ET, requiring an active nursing license and 2–4 years of RN experience in acute or ambulatory care settings.

Qualifications

• 2–4 years of full-time RN clinical experience.
• Active, unencumbered nursing license in good standing.
• Experience with telephone triage and electronic triage tools.

Responsibilities

• Respond to inbound/outbound calls for multi-state nurse triage.
• Document patient interactions accurately in client EHRs.
• Provide clinical assessments via phone or patient portal.
• Support providers with administrative/clinical tasks per protocol.
• Facilitate referrals and prior authorizations requiring clinical input.
• Collaborate with hospitals, nursing homes, and community partners.
• Participate in Remote Patient Monitoring programs (RPM).
• Screen and qualify patients for clinical trials.
