Job summary

Call 4 Health is seeking experienced Registered Nurses to join our 24-hour medical call center as Remote Triage Nurses. Work from home supporting patients and providers nationwide, with a focus on clinical assessments and timely triage via phone, portal, and EHR documentation.

This role offers flexible scheduling in ET, opportunities to develop telehealth skills, and collaboration with hospitals, clinics, and care teams to improve patient outcomes.

Qualifications

• Associate’s or Bachelor’s Degree in Nursing.
• 2–4 years of full-time RN clinical experience (ER/Urgent Care/Adult/Pediatric/Ambulatory/ICU).
• Active, unencumbered e-NCL / Enhanced Nurse Licensure Compact license.
• Willingness to obtain additional state licenses as requested.
• Experience with telephone triage and multiple EHR platforms.

Responsibilities

• Respond to inbound/outbound calls for multi-state nurse triage service.
• Use Schmitt-Thompson protocols to assess/triage patients.
• Document patient interactions in client EHR systems.
• Provide clinical assessments via phone or portal per protocols.
• Assist providers with administrative tasks and referrals.
• Coordinate post-discharge and post-operative follow-up.
• Facilitate referrals and prior authorization requests.
• Collaborate with hospitals, nursing homes, and agencies as needed.
• Participate in remote patient monitoring (RPM) programs.
• Screen and qualify patients for clinical trials and patient education.
• Take on additional responsibilities to support team and patient outcomes.
