Sagility is seeking an experienced Registered Nurse to join as a Senior Care Specialist in a work-from-home capacity. In this remote role, you will assess members, develop care plans, provide education, and coordinate services to support chronic and complex health conditions by phone.

Responsibilities include telephonic health assessments, care planning, health coaching, transitions of care, and collaboration with a multidisciplinary team to promote optimal health outcomes.

Formación

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and remotely.
• Intermediate to advanced computer proficiency (Word/Excel/Outlook).
• Ability to navigate multiple EMR systems.
• Strong verbal and written communication skills.

Responsabilidades

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs.
• Establish health coaching relationships and provide education and support.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning with DME and home health coordination.
• Help members navigate benefits and care options.
• Assess factors that affect health and well-being.
• Collaborate with social workers, dietitians, pharmacists and providers.
• Promote self-management with tailored education and resources.
• Coordinate care activities among members, caregivers, and providers.
• Document interactions, assessments, and outcomes accurately in systems.
