Job summary

Sagility is seeking an experienced Registered Nurse to join as a Senior Care Specialist in a work-from-home capacity. You will assess members by phone, develop individualized care plans, educate and support for chronic conditions, and coordinate services to improve health outcomes.

Responsibilities include telephonic assessments, care planning, health coaching, and collaboration with multidisciplinary teams while leveraging EMR systems and Microsoft Office in a remote setting.

Qualifications

• Active, unrestricted RN license in home state.
• Compact RN license permitting practice in applicable states.
• 2+ years clinical nursing experience; 3+ years preferred.
• Strong clinical judgment and problem-solving skills.
• Ability to work remotely and use multiple computer systems.
• Proficiency with Word, Excel, and Outlook.

Responsibilities

• Conduct telephonic health assessments for Medicare/Medicaid/group plans.
• Develop actionable, individualized care plans based on needs and risks.
• Provide health coaching, education, and support for chronic conditions.
• Use nursing judgment to coordinate interventions for acute situations.
• Support transitions of care and discharge planning with DME and services.
• Assist members in navigating benefits and care options.
• Assess environmental and psychosocial factors affecting health.
• Collaborate with social workers, dietitians, pharmacists, and providers.
• Promote self-management with education and resources.
• Document interactions and outcomes accurately in systems.
