Sagility seeks experienced Registered Nurses to join as Senior Care Specialists in a work-from-home capacity for a seasonal assignment ending June 2027. The role involves telephonic health assessments, care planning, and education to help members manage chronic conditions.

You will collaborate with care teams to identify risks, remove barriers, and promote better health outcomes using a Whole Person Care approach. Remote US-based candidates with RN licensure and compact licensure are preferred.

Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3 or more years preferred.
• Strong clinical judgment and problem-solving skills.
• Ability to work independently in a remote environment.
• Intermediate to advanced computer proficiency (Microsoft Word, Excel, Outlook).
• Ability to navigate multiple computer systems and EMR.

Responsabilités

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use clinical nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and discharge planning, including DME and home health services.
• Coordinate care with multidisciplinary teams and navigate healthcare benefits for members.
