Responsibilities

• Assists patients via phone and electronic platforms to determine care needs and schedule appointments.
• Provides general health information and education using approved references.
• Coordinates with Primary Care physicians and nursing staff to meet patient needs.
• Documents patient health status, care concerns, and nursing observations.
• Maintains accuracy of patient information in the EMR and medication reconciliation.
• Delivers high-quality patient care with attention to detail and professional conduct.
