Job summary

Carle Health seeks an RN to join the Patient Advisory team, with 6 months of in-office training in Champaign, IL before transitioning to remote work. You will triage patient concerns by phone/video, develop care plans, and coordinate care for patients around the clock.

Responsibilities include medication management, education, and cross-department support in a fast-paced environment, ensuring patient-centric, high-quality care.

Qualifications

• RN license in OH within 1 year (OBON).
• RN license in IA within 1 year (IBON).
• RN license in IN within 1 year (IPLA).
• RN license in IL (IDFPR) within 1 year.
• RN license in WA within 1 year (WABON).

Responsibilities

• Assesses and triages patient concerns via telephone/video/electronic methods using embedded adult and pediatric protocols.
• Assesses health needs, develops plans of care, and coordinates care for patients who are experiencing health concerns.
• Documents all telephone encounters in Epic and completes EMR documentation and follow up.
• Provides medication management assistance by renewing, refilling, cancelling, transcribing, and reconciling orders.
• Ensures adherence to standards of care and scope of practice; provides education and counseling to patients and families.
• Facilitates 24/7 care coordination between patient and provider; cross-trains to assist with multiple departments.
