Carle Health is seeking a Registered Nurse to support telehealth triage and care coordination. The role begins with six months of in-office training at our Champaign, IL location before transitioning to remote work. You will manage triage, documentation in Epic, and patient education across 24/7 operations.

Required are an RN license in applicable states within 1 year and a Nursing Diploma, with strong communication and critical thinking skills to deliver patient-centric care.

Qualifications

• Licensed RN in one or more listed states within 1 year.
• Diploma in Nursing required.
• Strong communication and triage skills.

Responsabilités

• Triage patient concerns via telephone/video/electronic channels using established protocols.
• Coordinate care plans and follow up with patients.
• Document encounters in Epic EMR and complete necessary documentation.
• Provide medication management assistance per guidelines.
• Support 24/7 operations and care coordination.
• Cross-train across Patient Contact Center departments.
