Job summary

Call 4 Health is seeking a Registered Nurse (RN) for a remote physician extender role in a multi-state telehealth team. You will manage Electronic Health Records (EHR), respond to patient inquiries, and provide timely clinical guidance to promote high-quality patient care.

Duties include triage by phone using established protocols, documenting interactions in client EHRs, coordinating follow-up care after hospital discharge, facilitating referrals, RPM participation, and supporting care

Qualifications

• Associate’s or BSN degree in Nursing.
• 2–4 years of full-time RN clinical experience.
• Active eNLC license required; other state licenses a plus.
• Experience with at least two EHR platforms.

Responsibilities

• Triage calls and document patient interactions using EHR.
• Provide clinical assessments per protocols via phone or portal.
• Coordinate follow-up care after hospital discharge or procedures.
• Support RPM programs and patient education on chronic diseases.
• Assist with referrals and prior authorization requests.
