Job summary

Guideway Care is seeking an experienced Registered Nurse for remote triage in a fast-paced, patient-centered environment. You will perform comprehensive assessments, triage using evidence-based protocols, and provide education and guidance to patients across ambulatory and primary care settings.

Key duties include handling calls, coordinating care with providers, and documenting in EMR systems. Remote role with flexible hours and a strong emphasis on HIPAA compliance and quality care.

Qualifications

• Active, unrestricted RN license in a compact state.
• Minimum 5 years in ambulatory/outpatient or primary care settings.
• Prior RN triage experience preferred.
• Previous remote nursing experience preferred.
• Strong clinical assessment and communication skills.
• Experience with EMR systems (Epic, Cerner, Athenahealth, Greenway).
• Proficient with MS Office and remote work tools.

Responsibilities

• Conduct patient assessments and triage using evidence-based protocols.
• Handle inbound/outbound calls and secure patient messaging.
• Assess symptoms and determine levels of care with education.
• Escalate urgent concerns to providers or care team.
• Coordinate care across clinicians to support outcomes.
• Document in EMR accurately and timely.
• Support onboarding and training of new team members.
• Maintain HIPAA compliance and quality standards.
