Job summary

Elevance Health is seeking a Nurse Disease Management I to participate in patient education and disease management interventions, providing health coaching for members across multiple lines. The role supports chronic disease improvement with a focus on behavioral health needs, in a hybrid/ virtual setup with in-person training sessions as required.

Qualifications

• Requires AS in nursing and RN license.
• Minimum 2 years of clinical or home health experience.
• BS in nursing preferred.

Responsibilities

• Conduct assessments to identify member knowledge and needs.
• Coordinate health coaching plans with providers.
• Educate members on clinical needs and facilitate referrals.
• Use motivational interviewing to support behavior change.
• Monitor effectiveness and modify interventions as needed.
• Direct members to facilities and appropriate care networks.
• Refer members to case management when needed.
