Elevance Health in Las Vegas, NV seeks a Nurse Disease Management I to deliver patient education, disease management interventions, and health coaching for members with chronic conditions. The role is full-time and primarily virtual with in-person training sessions.

Responsibilities include assessments, care planning, coordinating with providers, and utilizing motivational interviewing to drive health outcomes.

Qualifications

• AS in nursing required.
• BS in nursing preferred.
• Minimum of 2 years of condition-specific clinical or home health/discharge planning experience required.
• Current unrestricted RN license in applicable state(s) required.

• Identify and coordinate health coaching plan needs to address objectives identified during assessments.
• Interface with providers and other health professionals to coordinate health coaching plans.
• Educate members regarding clinical needs to implement and coordinate coaching and care plans.
• Facilitate referrals to health professionals for behavioral health needs.
• Use motivational interviewing to facilitate health behavior change.
• Monitor and evaluate the effectiveness of interventions and modify as necessary.
• Direct members to facilities, community agencies, and appropriate providers.
• Refer members to catastrophic case management.

About This Role

The Nurse Disease Management I is responsible for delivering patient education and disease management interventions, as well as performing health coaching for members to improve health outcomes for chronic diseases.

• Pay: $70,560 - $88,200/year

What You'll Do

• Conduct behavioral or clinical assessments to identify individual member knowledge, skills, and behavioral needs.
• Identify and coordinate specific health coaching plan needs to address objectives and goals identified during assessments.
• Interface with providers and other health professionals to coordinate health coaching plans for members.
• Implement and coordinate coaching and care plans by educating members regarding clinical needs.
• Facilitate referrals to health professionals for behavioral health needs.

Requirements

Education

• AS in nursing required; BS in nursing preferred.

Experience

• Minimum of 2 years of condition-specific clinical or home health/discharge planning experience required; prior case management experience preferred.
