Job summary

Elevance Health, Inc. is seeking a Telephonic Nurse Disease Management I to conduct assessments and coordinate health coaching plans for members. The role is primarily virtual with required on-site training sessions and occasional alternate locations within commuting distance.

You will coordinate care plans with providers, educate members on clinical needs, and apply motivational interviewing to drive behavior change. A valid RN license and nursing degree are required.

Qualifications

• AS in nursing or equivalent clinical background required or acceptable combination of education and experience.
• RN license in applicable state(s) required.
• BS in nursing preferred.

Responsibilities

• Conducts behavioral/clinical assessments to identify member needs.
• Identifies and coordinates health coaching plan objectives.
• Interfaces with providers to coordinate health coaching plans.
• Educates members and facilitates referrals for behavioral health needs.
• Uses motivational interviewing to support behavior change.
• Monitors interventions and adjusts plans as needed.
