Elevance Health is seeking a Telephonic Nurse Disease Management II to deliver complex patient education and disease management interventions across multiple chronic conditions. You will perform health coaching, coordinate care plans with providers, and guide members through behavior change using motivational interviewing.

The role offers a sign-on bonus with hybrid/remote work and requires RN licensure with 3+ years of experience; in-person training sessions are required as needed.

Qualifications

• Requires AS in nursing and minimum of 3 years of condition-specific clinical or home health/discharge planning experience; RN license in applicable state(s) required; BS in nursing preferred.
• Prior case management experience preferred.

Responsabilités

• Conducts behavioral or clinical assessments to identify member knowledge, skills and behavioral needs; coordinates health coaching plan objectives.
• Coordinates health coaching plan with providers and other health professionals.
• Educates members regarding clinical needs and facilitates referrals to health professionals for behavioral health needs; uses motivational interviewing.
• Monitors and evaluates effectiveness of interventions and modifies plans.
• Directs members to facilities, community agencies and appropriate provider/network.
• Serves as a resource to lower-leveled DM associates.
• Refer member to catastrophic case management as appropriate.
