Job summary

Elevance Health, Inc. seeks a Telephonic Nurse Disease Management I to deliver patient education and disease management interventions. This role supports health coaching for members across chronic diseases, with in-person training sessions and hybrid work arrangements.

Ideal candidates hold an RN license, AS in nursing, and 2+ years of relevant clinical/home health experience; BS in nursing is a plus. The position includes a sign-on bonus and a clear path for impact in FEP programs.

Qualifications

• Requires AS in nursing and minimum of 2 years of condition-specific clinical or home health/discharge planning experience.
• Current unrestricted RN license in applicable state(s) required.
• BS in nursing preferred; prior case management experience preferred.

Responsibilities

• Conducts behavioral or clinical assessments to identify member knowledge, skills and behavioral needs.
• Identifies and coordinates health coaching plan needs to address objectives identified during assessments.
• Interfaces with provider and other health professionals to coordinate health coaching plan for the member.
• Educates members regarding clinical needs and facilitates referrals to health professionals for behavioral health needs.
• Uses motivational interviewing to facilitate health behavior change.
• Monitors and evaluates effectiveness of interventions and health coaching plans and modifies as needed.
• Directs members to facilities, community agencies and appropriate provider/network.
• Refers member to catastrophic case management.
