Job summary

Elevance Health is seeking a Telephonic Nurse Disease Management I to deliver patient education and health coaching for chronic disease programs. The role supports flexible, virtual work with required in-person onboarding and periodic training sessions.

Responsibilities include assessment, coaching plan development, and coordination with providers to optimize member health outcomes. Strong RN background and home health experience are valued for success in this role.

Qualifications

• Requires AS in nursing and minimum 2 years of condition specific clinical or home health/discharge planning experience.
• Current unrestricted RN license in applicable state(s) required.
• BS in nursing preferred.

Responsibilities

• Conducts health coaching sessions and patient education.
• Identifies and coordinates health coaching plan needs to address objectives.
• Coordinates with providers to implement care plans and referrals.
• Uses motivational interviewing to support behavior change.
• Tracks outcomes and adjusts interventions as needed.
• Directs members to appropriate facilities and services.
