Qualifications

• Active RN license in the state

Responsibilities

• Perform telephonic outreach to members identified by clinicians for referral-related issues
• Ensure members have scheduled appointments with PCP or specialists
• Refer members to internal departments when appropriate
• Gather clinical information telephonically from patients/families
• Assist with urgent needs requiring acute intervention
• Provide education on disease processes and trigger management
• Maintain accurate documentation and HIPAA compliance
