Job summary

Lumicera Health Services is seeking a RN, Clinical Operations to deliver patient-centered telephonic education and clinical support for specialty therapies. The role focuses on clinical assessment, patient education, and adherence support while documenting care in accordance with regulatory guidelines.

Ideal candidates will hold an ADN with RN licensure and have 2 years of patient-centered care or disease management experience.

Qualifications

• Associate Degree in Nursing (ADN) required.
• Bachelor of Science in Nursing (BSN) preferred.
• Current, unrestricted RN license in the state of residence required, multi-state license preferred.
• 2 years of patient-centered care or disease management nursing experience preferred.

Responsibilities

• Perform clinical assessments and provide disease- and therapy-specific education to patients and caregivers.
• Identify patient needs related to therapy use, adherence, persistence, adverse effects, and disease-state concerns; triage to a pharmacist or notify the provider as appropriate.
• Accurately document assessments, interventions, care coordination activities, and notes in the patient record per requirements.
• Support development and maintenance of clinical programs and projects to improve patient care and outcomes.
• Complete required documentation including adverse event forms and program-specific forms per guidelines.
• Collaborate with pharmacists, clinical team members, manufacturers, and providers to support programs and patient care needs.
• Assist with order creation and setup activities, including payment methods and shipment coordination as needed.
• Maintain clinical competency across treatments and disease states supported by Lumicera.
• Ensure compliance with accreditation standards, policies, HIPAA, and regulatory requirements.
• Perform other duties as assigned
