Job summary

Lumicera Health Services is seeking a registered nurse for a telephonic education role, delivering patient-centered education and assessments to support specialty therapies. You will help patients understand their treatment plans, overcome barriers, and stay engaged in their care journey.

The RN, Clinical Operations role emphasizes clinical documentation, adherence support, and collaboration with pharmacists and clinicians in a remote setting, with a schedule of five 4-hour shifts per week.

Qualifications

• ADN is required.
• BSN preferred.
• Current, unrestricted RN license in state of residence.
• Minimum 2 years of patient-centered care or disease management nursing experience.

Responsibilities

• Conduct clinical assessments and provide disease- and therapy-specific education to patients and caregivers within nursing scope and guidelines.
• Identify and address patient needs related to therapy use, adherence, and adverse effects; triage to a pharmacist or notify provider when appropriate.
• Document clinical assessments and interventions in the patient record per regulatory and client requirements.
• Support clinical programs and projects that promote patient care, adherence, and improved outcomes.
• Collaborate with pharmacists, clinical team, and providers to support programs and patient care needs.
