Job summary

Titanium Healthcare is seeking an RN Clinical Consultant to lead clinical oversight within the Enhanced Care Management (ECM) program for Medi-Cal members in California. You will collaborate with Lead Care Managers and the interdisciplinary team to ensure clinically appropriate, compliant care and effective care planning.

Remote role with standard hours, you will oversee Comprehensive Assessments, Care Management Plans, Transitions of Care, and clinical escalations, supporting improved health

Qualifications

• Associate’s degree in nursing.
• Minimum 1+ years of experience as an RN in a healthcare setting.
• Current and unrestricted CA state RN license.
• Experience using an EHR.

Responsibilities

• Provide clinical oversight to ensure high-quality care management across the ECM program.
• Collaborate with Lead Care Managers and ancillary clinical staff to support patient care needs and clinical decision‑making.
• Facilitate access to primary care, behavioral health, and specialty services as needed.
• Oversee Comprehensive Assessments and Care Management Plans for accuracy and alignment with plan guidelines.
• Lead or participate in Clinical Case Conferences to address complex member needs.
• Engage in Interdisciplinary Care Team meetings to support coordinated care.
• Oversee medication reconciliation for safety and accuracy.
• Support Transitions of Care after ER visits or hospitalizations with timely follow-up.
